Prepare the following documents to accompany your registration submission.
- Child's Birth Certificate
- Proof of Residency (acceptable pieces of identification: current property tax bill, property assessment notice, tenant rental lease agreement, utility bill)
NOTE: diver's license, health card, phone/retail bill are not accepted - Proof of immigration status for non-Canadian born students, such as a stamp in a student's passport or other documentation such as plane tickets, boarding passes or other visa documentation that algin with the entry dates.
Families who are temporary residents in Canada (i.e., Work Permit, Study Permit) - Legal documents related to custody and access, if applicable
IMMUNIZATION REQUIREMENTS:
The Ministry of Education requires that every student have a complete and up-to-date immunization record on file with the local Health Unit in order to attend school. Contact Chatham-Kent Public Health or Lambton Public Health to ensure your immunization records are up-to-date.

All Catholic property owners and tenants are encouraged to ensure that they are registered appropriately for the purposes of municipal elections and school board tax support. This is also a requirement for admission to the St. Clair Catholic District School Board elementary schools.
